All new concierge members receive a full physical exam and comprehensive lab work in addition to a review of medical history with general intake. Upon enrollment patients are required to create a secure email account for correspondence with the DPS office.

Included in Annual Membership:

• Biennial (every two years) history review and physical exam with basic routine lab work, in the birthday month.

• Off year well check and review of care plan and progress, in the birthday month.

• A standing preventative schedule is provided as part of your primary wellness care

First Year Enrollment and Membership Pricing:

Ages 65> : enrollment + first year membership $970 / subsequent annual membership $820

40-64: enrollment + first year membership $825 / subsequent annual membership $675

30-39: enrollment + first year membership $710 /subsequent annual membership $560

18-29: enrollment + first year membership $485/subsequent annual membership $335
